Periodontal charting practice

Do not enter patient-identifying information. The chart asks for a universal tooth number. It does not ask for a name.

Each tooth can take six sites: mesiobuccal, buccal, distobuccal, mesiolingual, lingual, and distolingual. Clinical attachment level here is probing depth plus recession, where a positive recession is apical to the cementoenamel junction and a negative number means the margin sits coronal to it. Bleeding is a separate mark on that site.

The summary counts sites, bleeding points, depths of 4 mm and 6 mm or more, and the mean probing depth and mean attachment level. The chart stays in this browser. Printing and a text download are part of the paid pack. Entering sites and reading the summary are free.
